Process Safety Has a Human Dependency

Major accident hazard sites — refineries, offshore platforms, LNG terminals, pipelines — are engineered around layers of protection: containment, detection, shutdown systems, emergency response. But every one of those layers is designed, operated, maintained, or overridden by a person. Process safety frameworks like Seveso, OSHA PSM, and IOGP guidance all converge on the same point: technical barriers only hold if the people operating them are competent for the specific task, in the specific conditions, at the specific moment it matters.

Oil & gas has historically managed this through training matrices and permit-to-work systems. Increasingly, that’s not enough.

  • Permit-to-work depends on competency verification at the point of authorisation. Before a hot work or confined space permit is signed, someone needs to confirm — not assume — that everyone involved holds the current, relevant competency for that specific task and environment.

  • Contractor-heavy workforces. Turnarounds, shutdowns, and major capital projects bring in large contractor populations, often from multiple tiers of subcontractors, each needing verification against site-specific competency requirements before mobilisation.
  • Ageing workforce, ageing assets. Many facilities are operated by a workforce nearing retirement, running assets that are themselves decades old. The tacit knowledge of how a specific piece of legacy equipment actually behaves is a real and under-tracked risk.

What Regulators and Investigators Look For

Major incident investigations — from Piper Alpha to Deepwater Horizon to Texas City — have repeatedly identified competency and supervision gaps as contributing factors, not just equipment failure. Regulators now expect operators to demonstrate active, ongoing competency assurance as part of their safety case, with clear evidence of who was authorised for what, and how that was verified, before an incident — not reconstructed after one.
